Outside Interests Disclosure Form

Faculty, Staff, Postdocs, Residents, and GTAs, GRAs, and GAs – Complete New Outside Interest Disclosure (OID) Form by May 31!

All regular faculty, staff, postdocs, residents, and GTAs, GRAs, and GAs must complete the OID form. If you completed a new OID form on January 1, 2024 or after, you do not need to submit a new one unless you have a change to report.

Complete and Submit your Disclosure for Fiscal Year 2023-24:

  1. Please sign into the Employee Self-Service tab with your UT NetID and password. 
  2. Under the Personal Information area, choose the Outside Interest Disclosure link and follow the instructions on the screen. Note: If the OID form does not launch, please change your browser settings to allow “pop-ups and redirects.”
  3. Employees can use the Help and Video buttons at the top right of the screen for additional information or to watch a brief video explaining the form.

You can use the View Previous Forms button at the top right of the screen to see the list of prior forms you have filled out, including the dates and the status of each form. If your previous OID form has not been approved, you will not be able to complete a new one until your supervisor approves your previous form. After completing and submitting the form, it will be routed electronically through the review and approval process as set forth in the Conflicts of Interest and Commitment Policy.

Supervisor Approval Process

The OID form routes to the IRIS Inbox of the employee’s supervisor for approval. The supervisor receives a system generated email notification that an OID work item is pending their review.

The IRIS Inbox is available at: irisweb.tennessee.edu

  1. Use your NetID and password to log in.
  2. Once you are logged in, select the Inbox tab.
  3. The Inbox screen is displayed with current work items pending approval.
  4. Click on the OID form you want to review from the Workflow items. Note: If the OID form does not launch, please change your browser settings to allow “pop-ups and redirects.”
  5. Click on OID Form button at the top left to review it. Exit the form.
  6. Answer the questions shown in the box and Click on the APPROVE button.

Note to Business Contacts: You can use IRIS Transaction ZPR_OID_REPORT to determine who has not completed the OID Form in your department.
