Ordering electronic transcript from the National Student Clearinghouse

Ordering on Banner Self Service

1. To order an official transcript, login to UTHSC Banner Self-Service at this link.
2. After logging in, select “Student,” “Student Records,” and “Request Academic Transcript (Official)” or “Print Unofficial Transcript (Web Version)”
3. Follow instructions to enter transcript request information.
4. After your order has been submitted, it will be processed within two business days.

1. To order an official transcript, login to the National Student Clearinghouse.
2. Follow the instructions for ordering and paying for your transcript.
3. You will receive email or text updates on the status of your request.

2. Complete your request and upload any documents to be sent with the transcript.
3. After your order has been submitted, it will be processed within two to three business days.

1. To request an ERAS (Electronic Residency Application Service) or VSLO transcript complete the request form found at this link.
2. Once your request has been submitted, it will be processed within two to three business days.

Current students and alumni requesting an educational verification letter or form to be completed, please find the request form.  If you have a form to be completed, you will be able to upload it to your request.  There is no charge for this service.

Duplicate / Replacement Diplomas:

To order a duplicate or replacement diploma, please click here to place your order though Paradigm, our diploma servicer. Placing your diploma reorder online via Paradigm is the only accepted method for diploma replacement.

All Diplomas will reflect four (4) signatures from the current UT-System administrative officials.

Timeline / Delivery Window: 

Domestic (non-expedited): 6-8 weeks
Domestic (expedited): 3-4 weeks

Notification of shipment will be emailed to the email address used on the order when the diploma is produced and shipped. 

Name changes on Diploma: You will need to provide a copy of a legal document showing the name requested. This is only if the name differs from what appears on the academic transcript. The "Name Change On Academic Record Form" is located here. 

Certified copy of diploma (CeDiploma) / Certified Electronic Credentials (CeDiploma or CeCertificate) 

+ The Certified Electronic Diploma (CeDiploma) is an electronic version of your diploma. The diploma information is presented in a secure, portable, and verifiable format.
