Exit Clearance Packet
This is the exit clearance packet. Please ensure you pick the correct version of each file, specific to your location. Additionally, please read all instructions carefully, to avoid any hangups with the exit process.

Important Notes
Please clear with all hospitals you are finished rotating with first. Also, Methodist/Le Bonheur are counted as the same entity regarding the exit process, so do not clear one while still rotating at the other.
For your remaining sites, wait until your final working shift to finalize the exit with them. Some hospitals will terminate your access (both hospital and EMR) immediately upon exiting, and we want to ensure you aren't prematurely locked out of buildings/systems needed for charting or patient care.
Please do not attempt to come to GME to finalize your exit until you are fully prepared to turn in all completed documents, badges, and parking passes. If you have a hangtag for the 910/920/930 garage (H-Lot), you must turn that in as well.
GME Office Info & Hours for Exiting
Exits will take place from Monday through Fridays, between 8:00 AM and 4:00 PM in the 920 Madison building on the 5th floor. There will be blue signs posted, directing you to our exit station.
What is in the Attached Packet?

- What it is: Explains important details and specific instructions on how to clear with individual hospitals.
- Do you turn it in? No. Keep this for your records.
- UTHSC GME Exit Clearance Form (M).pdf
- What it is: This must be filled out entirely.
- Hospital Signoffs: If you did not work at one of the listed hospitals on page 2, simply write "N/A" next to that hospital's name. When clearing with hospitals, please include all confirmation emails concerning your hospital exits.
- Badges/Parking: You will turn in all hospital badges and your parking pass (if you were given an H-Lot pass) to us—except for the VA, as they collect their own badges.
- Signatures: Your Program Director or Program Manager must sign page 3. GME personnel will fill out the bottom of page 3 upon your final exit.
- Do you turn it in? Yes. This must be turned in.

- What it is: Your official letter explaining your professional liability coverage. Graduated residents and fellows frequently request this, years down the road, so please save a copy where you won't lose it.
- Do you turn it in? No. This is for your permanent records.
- NPI-Directions.pdf
- What it is: Instructions on how to update your National Provider Identifier (NPI) information.
- Important: If you do not have a new employer or school lined up yet, update the NPI to your personal contact information. In the past, trainees left our office number on their NPI, meaning pharmacists and patients trying to reach them years later couldn't get ahold of them.
- Do you turn it in? No, but you must provide printed/digital proof that you successfully updated the information (as explained in the document) when you exit.

- What it is: Instructions on how to update your address in the DASH system to avoid having issues with receiving your W-2.
- Important: If you do not have a new address yet, update your address to a relative's address so you can receive your W-2 without issues.
