Community Health Worker Certification Program

Prerequisite: You must have completed the Motivational Interviewing Certification Program and be certified at level 1 or higher to sign up for this program.

 

Who:  This program is designed primarily for individuals with at least a high-school education who seek to work full or part-time in a clinical or community setting delivering patient navigation and/or care coordination services.

What:  An interactive, self-paced program that provides an overview of best practices of self-management prescribed for the most common chronic conditions, and evidence-based lifestyle management practices, emphasizing patient navigation fundamentals that qualifies you to help patients navigate the healthcare system, overcome barriers to needed care, and get the care they need most.

When:  Available soon.

Where: 100% online; complete at your own pace/convenience.

How: Lectures, practice activities, feedback on assignments, and case study competency assessment.

Requirements for Successfully Completing the Course:   Completion of modules, satisfactory completion of assignments, and passing score on competency-based activity.

Certification levels provided:  All those completing course requirements will be certified as a Level 1 (Client-centered), Level 2 (Competent), or Level 3 (Proficient) CHW based on their completed MI competency assessment.

Estimated time required: 12 hours

Fee: $495 – Includes online training modules and competency assessment. A 50% discount is available for Tennessee Population Health Consortium Members and Partner Organizations. This training is provided for free for two new or existing employees of practices that are fully participating in the TN Heart Health Network study. Email sbutter1@uthsc.edu for more details.
