Enrolling for an Affiliate Membership with UT Health Science Center's Campus Recreation
is fast and easy! Follow the steps below to submit your application:

Fill out Membership Enrollment Form completely

Fill out the Information/Non-UT Application

Select Alumni/Affiliates and either 1 year or 6 months

Sign the Primary Member Signature

Sign the Usage Agreement

Upload Work Picture ID and Driver's License

Once approved, the QR code for payment will be sent to you.
