  • Dr. James Sallis has a website of downloadable physical activity and dietary questionnaires that you may find helpful in your current or future research. These include instruments dealing with social support, self-efficacy, environment, as well as parent and child behaviors. Dr. Cowan used the 7-day Physical Activity Recall instrument in her research studies involving adolescents and adults. Here is the website: http://sallis.ucsd.edu/.
